Important advice for your stay at the hospital

Diagnosis
If existing: blood donor card
Indigestibility or allergy ID
If available: patients’ admission form
Complementary insurance with policy number, if you are a private patient
Social security number
Addresses/telephone numbers of: relatives or attorney, family doctor, medical specialist

Details about:

Childhood diseases, former diseases, surgery, hereditary illnesses
Medication which you have to take constantly (empty boxes)
For women: pregnancies and births

For your personal needs:

Dental hygiene items, toiletries, shaving equipment, tissues, slippers, nightdresses/nightshirts/pyjamas, dressing gown, tracksuit, underwear, socks/stockings, if necessary: glasses


 

Before your hospital stay please:

NotiNotify appropriate authorities that you cannot meet due dates because of your stay in hospital (e.g.: at the post office: radio, TV, telephone bill)

Inform relatives and your employer about your hospital stay (with address and room number for postal delivery)
Ensure care for pets, plants,... by reliable persons
If you have a complementary insurance: inform your private insurance company
In case of a longer stay: give instructions for mail forwarding
